Workplace Safety Templates
Occupational health and safety law asks employers to prove what they did: that hazards were identified and assessed, that controls were put in place, that workers were told about them and that the assessment was repeated when the work changed. Inspectors ask for these records first, and after an incident they are the evidence of due diligence. Each jurisdiction's legislation sets out what the records must contain and when they must be updated.
The forms in this category are written to a named jurisdiction's OHS legislation and regulator guidance. Each one follows the steps the regulation sets out, provides space for worker participation and sign-off where the law requires it, and explains each column or field in a short guidance note. Completed examples show the level of detail an inspector expects.
These forms support a safety program; they are not the program itself. Involve workers and your joint committee or safety representative, and keep completed forms where workers can read them.
