Affidavit and Declaration Templates

Statutory declarations and affidavits are sworn or solemnly declared statements of fact used by courts, government agencies, insurers, lenders and employers. Their form is set by law: the wording of the declaration, who may take it and how the signature must be witnessed. A document that does not follow the prescribed form, or that is signed without a commissioner or notary present, will usually be rejected.

The templates here follow the form prescribed by the statute that applies. Each one sets out the declarant's details, numbered paragraphs of fact, any exhibits and the jurat in the correct wording, with guidance notes on writing facts clearly, attaching exhibits and finding someone authorized to take the declaration. Shaded fields mark what to complete.

Making a false statutory declaration or affidavit is an offence. Write only what you know to be true, and if the agency receiving it has its own form, use that form instead.
